DIRECTIONS & HOURS
Our hours are M-Thu 9am-5pm & 9am Fridays as needed
Please visit us in West Kalamazoo:
7110 Stadium Drive
Kalamazoo, MI 49009
Routine and annual appointments are generally scheduled a number of weeks in advance.
Urgent needs can be accommodated within a day or two, and true emergencies are seen immediately.
If an urgent or emergency visit is necessary, you may be seen by someone other than your usual health care provider.
We strongly encourage the presence of at least one family member at pre-operative consultations.
Once you've scheduled your first appointment you will be given login credentials for your electronic medical record. We ask that you register and login to Your Health File and enter all of your medical information. Having your full medical history will help us to provide you the best care possible.
We also ask our patients to come in at least 10 minutes prior to their appointments and to cancel appointments whenever possible with at least 24 hours notice. In return, we work very hard at running on time too. Sometimes our practice has urgent or emergency appointments come in, and even though we leave time in our schedules to accommodate such problems, we will sometimes be unavoidably late. We appreciate your patience and understanding when this happens. We try as best we can to minimize such lateness. We reserve the right to charge for missed appointments without 24 hours notice.
To contact us or TO MAKE AN APPOINTMENT, please call 269-372-7600 or use the online "contact us" form and we will get back to you as soon as possible. You can also email us at firstname.lastname@example.org . We look forward to hearing from you.
BILLING AND INSURANCE
We participate with many local and national insurance plans, but it is your responsibility to understand whether your insurance has limits on the doctors you can see or the services you receive.
Insurance plans are negotiated between insurance companies and employer groups; therefore everyone’s medical coverage is not the same. Individual coverage may require coinsurance, copays, or deductibles.
We require that these amounts due be paid at the time of service, as per your insurance carrier contract. Please bring your current insurance card to every office visit so that we may verify your insurance coverage.
Our practice participates with most insurance plans. Please check with your insurance company for exact coverages.
Co-pays are a contract between you and your insurance carrier and are due at the time of service. Some contracts require more than one co-pay on the same date of service if you have two different types of exams or procedures done.
If your insurance requires pre-approval or referrals to our practice, it is your responsibility to make sure that referral has been made.
We will complete one set of insurance forms for you. Please allow 7-10 business days for completion of disability, compensation or insurance forms.
Family Medical Leave Act (FMLA) Forms
We will complete one set of FMLA forms for you. Please allow 7-10 business days for completion of these disability or compensation forms.
Please speak with our billing staff if you have any questions about your services. We want to work with you to avoid any financial problems and to assure that your account remains in good standing.
You may call us anytime or use Your Health File to request a prescription refill. Your request will be handled within 24-48 business hours.
